About The Position

MPAC Healthcare is seeking Licensed Clinicians (LCPC, LMFT, LCSW) to set a new standard for quality of care for seniors. We are looking for team-oriented, reliable professionals driven by integrity to provide high-quality care in the senior care space. The Licensed Clinician role allows clinicians to function autonomously, similar to a private practice, without the administrative burden of managing a back office. The majority of your time will be dedicated to clinical work, with the remainder focused on patient documentation and facility correspondence. You will serve as an on-site clinical provider working with residents and their families, functioning as a lead member of an interdisciplinary team of physicians and nurse practitioners to address residents’ emotional, social, and psychological needs. You will have control over your caseload, schedule, and salary, providing initial assessments, utilizing psychotherapy, and offering ongoing support. Responsibilities also include all documentation requirements and staying updated on Medicare Benefit and commercial insurance plans.

Requirements

• Master’s degree
• Valid LCSW/LMFT/LCPC license in the state for which you are applying.
• Two years of counseling and therapy experience
• Ability to work as a member of a team with a great attitude
• Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches
• Experience collaborating with interdisciplinary teams, including other MPAC providers, to ensure patients are getting the best care possible
• The ability to foster open communication with the facility team and your Regional Clinical Manager to ensure the success of clinical programs
• Ability to take initiative and effectively communicate in multiple settings with multiple parties (patient, patient’s family, nursing staff, facility social services team, other MPAC providers, etc).
• High level of organization to manage patient needs, document effectively and ensure the mental health needs of residents are being met.
• Comfortable with the ever-changing environment of a skilled nursing facility.

• Nursing home/ hospital facility experience

Responsibilities
