About The Position

MPAC Healthcare is seeking Licensed Clinicians (LPC, LMFT, LCSW) to join their team and help set a new standard for quality care for seniors. The role offers the autonomy of a private practice without the administrative burden. Clinicians will function as on-site providers, working with residents and their families, and serving as active members of interdisciplinary teams to address emotional, social, and psychological needs. The position involves providing initial assessments, utilizing psychotherapy, offering ongoing support, and managing documentation requirements, including staying up-to-date on Medicare Benefit and commercial insurance plans. Requirements

  • Master’s degree
  • Valid LCSW/LMFT/LPC license in the state for which you are applying.
  • Two years of counseling and therapy experience
  • Ability to work as a member of a team with a great attitude
  • Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches
  • Experience collaborating with interdisciplinary teams
  • Ability to take initiative
  • High level of organization
  • Comfortable with the ever-changing environment of a skilled nursing facility

Responsibilities

  • Function as an active, lead member on an interdisciplinary team of physicians and nurse practitioners addressing residents’ emotional, social, and psychological needs.
  • Provide initial assessments, utilize psychotherapy to address mental health issues and offer ongoing support.
  • Responsible for all documentation requirements and keeping up-to-date on Medicare Benefit and commercial insurance plans.
  • Manage patient caseload and schedule.
  • Communicate effectively with patients, families, nursing staff, facility social services teams, and other MPAC providers.
  • Collaborate with interdisciplinary teams to ensure patients receive the best care possible.
  • Foster open communication with the facility team and Regional Clinical Manager.
  • Manage patient needs and document effectively to ensure mental health needs are met.
