About The Position

MPAC Healthcare is seeking Licensed Clinicians (LPC, LMFT, LCSW) who are passionate about improving the quality of care for seniors. This role offers the autonomy of a private practice without the administrative burden, allowing clinicians to focus on patient care. You will be an on-site provider working with residents and their families, functioning as a key member of an interdisciplinary team to address emotional, social, and psychological needs. The position involves providing initial assessments, utilizing psychotherapy, offering ongoing support, and managing all documentation requirements, including staying updated on Medicare and commercial insurance plans. Requirements

• Master’s degree.
• Valid LCSW/LMFT/LPC license in the state for which you are applying.
• Two years of counseling and therapy experience.
• Ability to work as a member of a team with a great attitude.
• Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches.
• Experience collaborating with interdisciplinary teams.
• Ability to take initiative and effectively communicate in multiple settings with multiple parties (patient, patient’s family, nursing staff, facility social services team, other MPAC providers, etc).
• High level of organization to manage patient needs, document effectively and ensure the mental health needs of residents are being met.
• Comfortable with the ever-changing environment of a skilled nursing facility.
• Entrepreneurial spirit.
• Strong medical knowledge.
• Understanding of documentation (CMS, Medicare, Medicaid).
• Nursing home/hospital facility experience.

Nice To Haves

• Knowledge of the Medicare Benefit and commercial insurance plans.

• Experience in the nursing home space is a plus.

Responsibilities

• Function as an on-site clinical provider working with residents and their families.
• Act as a lead member on an interdisciplinary team of physicians and nurse practitioners addressing residents’ emotional, social, and psychological needs.
• Provide initial assessments.
• Utilize psychotherapy to address mental health issues and offer ongoing support.
• Be responsible for all documentation requirements.
• Keep up-to-date on Medicare Benefit and commercial insurance plans.
• Manage your caseload and schedule.
• Collaborate with interdisciplinary teams, including other MPAC providers, to ensure patients receive the best care possible.
• Foster open communication with the facility team and your Regional Clinical Manager.
