About The Position

MPAC Healthcare is seeking Licensed Clinicians (LPC, LMFT) to provide high-quality care to seniors. This role offers the autonomy of a private practice without the administrative burden, allowing clinicians to focus on patient care, documentation, and facility correspondence. You will be an on-site provider working with residents and their families, acting as a key member of an interdisciplinary team to address emotional, social, and psychological needs. The position involves providing initial assessments, utilizing psychotherapy, offering ongoing support, and managing all documentation requirements, including staying up-to-date on Medicare Benefit and commercial insurance plans. Requirements

  • Master's degree
  • Valid LMFT/LPC license in the state for which you are applying
  • Two years of counseling and therapy experience
  • Ability to work as a member of a team with a great attitude
  • Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches
  • Experience collaborating with interdisciplinary teams
  • Ability to foster open communication with the facility team and your Regional Clinical Manager
  • Ability to take initiative
  • High level of organization to manage patient needs and document effectively
  • Entrepreneurial spirit
  • Strong medical knowledge
  • Understanding of documentation (CMS, Medicare, Medicaid)
  • Nursing home/hospital facility experience

Responsibilities

  • Provide initial assessments
  • Utilize psychotherapy to address mental health issues
  • Offer ongoing support to residents and their families

  • Function as an active, lead member on an interdisciplinary team

  • Manage caseload and schedule
  • Communicate effectively with patients, families, nursing staff, facility social services, other MPAC providers, and Regional Clinical Manager
