About The Position
This is a contract position for a Licensed Clinical Social Worker (LCSW) in an Acute Care setting. The role involves discharge planning and care coordination. The position is at a Level II Trauma Center with 571 total staffed beds across two campuses. The charting system is EPIC, and parking is facility-paid. The shift is 5x8 Days, and the contract length is 13 weeks. This role requires working every other weekend and may involve floating between units and campuses.
Requirements
- Master of Social Work (MSW) degree.
- Valid Washington State LCSW license.
- Minimum of 1 year of experience in Acute Care.
- Experience in Discharge Planning/Care Coordination.
- BLS certification.
- Must have a WA license.
- Must be able to work every other weekend.
Nice To Haves
- Diverse Case Management experience.
- Experience in Med Surg, LLOS, or ED.
Responsibilities
- Provide social work services in an acute care setting.
- Perform discharge planning and care coordination.
- Work in all areas of the hospital, including both Colby and Pacific Campuses.
- Float between like units as needed.
