About The Position

Clinical Social Workers in the Transition of Care Department handle disposition planning, Social Determinants of Health (SDOH), domestic violence, substance abuse, and homelessness. This specific position will specialize in SNF placement and home health/DME arrangement for adult Neurology (primarily Stroke and Seizure) patients. There are opportunities to assist with other patient populations, however the job remains the same.

Requirements

• Minimum 1 year of experience required
• BLS required
• NC licenses required
• EPIC proficiency

Nice To Haves

• Previous experience as an LCSW in a psychiatric or behavioral health setting

Responsibilities

• Handle disposition planning
