About The Position

Optimal Care is hiring a licensed clinical social worker to provide home health psychosocial support to patients and families in Grand Rapids and surrounding areas. The role focuses on helping patients address social and emotional barriers to recovery, follow medical treatment plans, access community resources, and work toward independence and wellness. The social worker collaborates with nurses, therapists, physicians, and other care team members to support patient outcomes. The position requires daily travel to patient homes and residential facilities within the service area, with occasional office meetings.
