About The Position

MTC Care is hiring a Licensed Clinical Social Worker to provide behavioral health services in long-term care facilities. This patient-first role supports under-resourced care teams and offers an established caseload, without administrative tasks. The position is listed as hybrid and 32 hours per week, with compensation prorated from the full-time rate. The social worker will provide individual and group psychotherapy, assess patients, develop treatment plans, educate facility staff, collaborate with interdisciplinary teams, and maintain compliant documentation.

Requirements

• Master's Degree in Social Work.
• Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Social Worker (LCSW) with the ability to practice and bill independently.
• Active license to practice in the state or states where MTC Care operates.
• Significant experience treating adult and/or senior patients, particularly patients exhibiting severe persistent mental illness.
• Strong communication skills and cultural humility.

Responsibilities

• Serve as a seasoned social work consultant in long-term care facilities.
• Provide individual and group psychotherapy sessions.
• Complete mood and symptom-based screenings.
• Develop individualized treatment plans for adults and/or seniors.
• Provide in-service education.
• Collaborate with facility staff and consult with interdisciplinary teams.
• Maintain accurate, compliant documentation.
