Licensed Clinical Social Worker/Licensed Professional Counselor
About The Position
At the Community Health Center of Central Wyoming (CHCCW), we believe that behavioral health is an essential part of whole-person care. We're seeking a compassionate, collaborative, and dedicated Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to join our integrated healthcare team and help improve the lives of individuals and families throughout our communities. Requirements
- Master's Degree in Social Work or Master’s Degree in Counseling from an accredited institution.
- Current Wyoming Licensed Clinical Social Worker (LCSW)/Licensed Professional Counselor (LPC) license or license-eligible in Wyoming.
- Experience working in a medical or integrated healthcare setting preferred.
- Experience conducting biopsychosocial assessments and developing treatment plans.
- Experience providing counseling across diverse populations and age groups.
- Passionate about integrated, patient-centered healthcare.
- Skilled at working independently while collaborating effectively with a multidisciplinary team.
- An excellent communicator with strong clinical judgment.
- Comfortable serving patients from diverse cultural and socioeconomic backgrounds.
- Knowledgeable about the connection between physical and behavioral health.
- Committed to providing compassionate, trauma-informed care.
Responsibilities
- Provide comprehensive behavioral health assessments, diagnostic interviews, and biopsychosocial evaluations.
- Develop individualized treatment plans and provide evidence-based counseling services.
- Diagnose behavioral health conditions using DSM-5 criteria.
- Provide individual and, when appropriate, group counseling services.
- Assist patients experiencing anxiety, depression, ADHD, substance use disorders, relationship concerns, trauma, and other behavioral health needs.
- Coordinate care and case management to ensure continuity of services.
- Collaborate closely with primary care providers and other healthcare professionals to integrate behavioral and physical healthcare.
- Provide age-appropriate behavioral health services for pediatric, adolescent, adult, and geriatric patients.
- Maintain accurate, timely, and confidential clinical documentation.
- Support compliance with CHCCW clinical policies, procedures, and quality standards.
- Participate in departmental meetings, case consultations, and quality improvement initiatives.
