Requirements

Master’s degree in Social Work (MSW).
Graduate from a School of Social Work fully accredited by the Council on Social Work Education (CSWE).
At least 12 months of experience, in the last 24 months, with assessing and providing evidence-based individual counseling to patients within a mental health or health care facility.
Active, unrestricted license to practice as a Clinical Social Worker from any state.
Current AHA or ARC BLS Healthcare Provider certification (may obtain upon acceptance, prior to start date).
Ability to pass a Government background check/security clearance.

Nice To Haves

A 4-day per week compressed schedule may be available upon approval.
Limited On-Call Duty to average 1 week every 90 days.

Responsibilities

Serve as a member of the primary care clinic’s health care team and assist the primary care manager (PCM) in managing the overall health of their enrolled population.
Improve management of cognitive and behavioral factors impacting the health of the enrolled population by assisting the Air Force Medical Home (AFMH) in meeting its core principles of "whole person orientation" and "coordinated/integrated patient care".
Provide patient care, including functional assessment, targeted and collaborative problem solving, and consultation with AFMH members to identify primary problems impacting the enrolled population.
Function as a full-time equivalent provider as an internal behavioral health consultant (IBHC) and/or Behavioral Health Care Facilitator (BHCF) in a patient-centered medical home (PCMH) location.
Participate in and successfully complete Air Force IBHC training (paid), including Phase I and Phase II.
Provide classes in formats appropriate for primary care, such as stress management, sleep enhancement, and chronic pain classes.
Participate in group medical appointments, including Shared Medical Appointments (SMA) and Drop in Group Medical Appointments (DIGMA), as requested by the PCM.
Work independently and adhere to Air Force values, using appropriate communication for addressing and resolving professional issues.
Communicate skillfully and tactfully with patients who may be physically or mentally ill, uncooperative, fearful, emotionally distraught, and occasionally dangerous.
Possess organizational, problem-solving, and communication skills to articulate medical requirements to patients, families/caregivers, and medical and non-medical staff.
Consult with PCMs on-demand about general issues and specific patients, ensuring same-day verbal and/or written feedback on referred patients.
Deliver brief presentations/trainings to primary care team members regarding behavioral health issues and IBHC services.
Participate in team efforts to develop, implement, evaluate, and revise clinical pathway programs.
Provide technical direction and clinical insight into relevant projects.
Provide concise documentation of care and recommendations in the patient’s electronic medical record within the allowable timeframe.
Effectively manage a full daily patient load, completing approximately 8-14 patient encounters per day.
Supervise providers who are in training or not fully licensed, as needed.
Participate in Air Force-approved research projects relating to Primary Care Behavioral Health.
Consult with medical personnel, legal authorities, and military commanders as required.
Participate in military-specific training (e.g., suicide risk assessment, pre-post deployment screening, PTSD, combat stress).
Attend and participate in meetings during normal duty hours and professional staff conferences, including quality improvement meetings, commander's staff meetings, and primary care staff meetings.
Participate in and successfully complete Air Force care facilitator training and demonstrate core competency skills within 240 days.
Contact referred patients to assess medication treatment adherence for depression, anxiety, and PTSD disorders.
Use validated brief symptom inventories to assess current symptom severity.
Assess barriers to treatment adherence and help patients problem-solve solutions.
Provide patients with education regarding their behavioral health condition and treatment regimen.
Maintain patient information in a patient registry.
Maintain accurate and current notes in the electronic medical records of all patients contacted for review by the managing physician.
