About The Position

The Licensed Case Manager for the Rehabilitation Unit is responsible for coordinating patient care to ensure appropriate utilization of resources, effective service delivery, and compliance with regulatory and payer requirements. This role provides comprehensive case management through assessment, care planning, implementation, and ongoing evaluation of individual patient needs, while incorporating patient preferences and promoting optimal outcomes. The Case Manager serves as a patient advocate, supporting care coordination, discharge planning, and continuity of care across the interdisciplinary team. This position focuses on enhancing the patient experience, improving quality outcomes, and supporting cost‑effective care throughout the rehabilitation stay. The Case Manager collaborates closely with clinical teams, patients, and families to ensure safe, timely, and well-coordinated transitions of care, while aligning with the organization’s mission, vision, and values.

Requirements

  • Current Registered Nurse (RN) or Social Work licensure or Healthcare professional licensure as Physical Therapist (PT), Speech Language Pathologist (SLP) or Occupational Therapist (OT).
  • Minimum of 2 years social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred.

Nice To Haves

  • Certification in Case Management or Rehabilitation Nursing preferred; for example, Commission for Case Manager Certification (CCM); Association of Rehabilitation Nurses (ARN) certification, American Case Management Association (ACMA) or Board Certification in CM by the ANCC e.g.: RN-BC
