About The Position

The Case Manager is responsible for coordinating social services and discharge planning to support patients throughout their continuum of care. This role assesses patient and family needs, identifies and arranges appropriate community and healthcare resources, and collaborates closely with providers, nursing, and interdisciplinary teams to ensure safe, effective care transitions. The Case Manager supports inpatient, outpatient, and Emergency Department populations while promoting compliance with regulatory standards, patient safety initiatives, and performance improvement efforts. This position requires strong critical thinking, sound judgment, and the ability to work independently in a fast-paced healthcare environment.

Requirements

• Bachelor's Degree Required
• Current State Social Work license
• Minimum of 3 years social work or case management experience
• Basic Life Support (BLS) within 60 days of hire

Nice To Haves

• Master's Preferred
• inpatient setting, acute/rehabilitation hospital experience preferred
