About The Position

Henry Ford Health is seeking an enthusiastic counselor to join a team of providers in a Family Practice setting. This role involves conducting patient assessments, establishing therapy goals, delivering individual and group therapy sessions, maintaining confidential records, tracking therapy progress, connecting patients with community resources, and collaborating with patients, families, physicians, and community partners to provide integrated, whole-person care. The position is onsite in Southfield with a shift of 8am-4:30pm.

• Conduct thorough patient assessments and work collaboratively to establish personalized therapy goals that spark meaningful progress and sustainable recovery.
• Deliver impactful individual and group therapy sessions while maintaining meticulous, confidential records that uphold clinical excellence and quality care standards.
• Track and celebrate meaningful therapy progress by evaluating intervention effectiveness and strategically adjusting your approach to drive optimal outcomes.
• Empower patients by connecting them with community resources and therapeutic activities that extend healing beyond the counseling room and strengthen their daily lives.
• Collaborate seamlessly with patients, families, physicians, and community partners to deliver integrated, whole-person care—from initial assessment through successful discharge and ongoing support.
