About The Position

The Home Health Licensed Practical Nurse provides one-to-one care and monitoring to patients in their homes, using clinical knowledge and technology. The nurse works collaboratively with physicians, rehabilitation team members, social workers, and home health aides in a multidisciplinary approach. The role requires someone who can work autonomously and is motivated, detail-oriented, organized, and technologically proficient. Coverage area includes Coopersville, Nunica, Bailey, and Ravenna.

Requirements

• Graduate from an accredited or state-approved school of practical nursing.
• Current licensure in the State of Michigan.
• Current CPR certification.
• At least one year of experience in an acute care setting.
• Current driver's license and reliable transportation to and from the work site.
• Ability to excel in an autonomous work environment; be highly motivated, detail-oriented, organized, and technologically proficient.
