About The Position

Trinity Health At Home – Muskegon is hiring a Home Health Licensed Practical Nurse to provide one-to-one care and monitoring to patients in their homes in the Grand Haven, Spring Lake, and West Olive coverage area. The LPN uses clinical knowledge and technology to deliver exceptional, compassionate care as part of a multidisciplinary team that collaborates with physicians, rehabilitation team members, social workers, and home health aides. The position requires an autonomous, motivated, detail-oriented, organized, and technologically proficient nurse.

The agency is a faith-based, not-for-profit home care, hospice, and palliative care organization serving communities across 11 states. Its services include skilled nursing, physical, occupational, and speech therapy, and medical social work. Requirements

• Graduate from an accredited or state-approved school of practical nursing.
• Hold current licensure in the State of Michigan.
• Have current CPR certification.
• Have a minimum of one year of experience in an acute care setting.
• Have a current driver's license and reliable transportation to and from the work site.
• Be motivated, detail-oriented, organized, and technologically proficient, and excel in an autonomous work environment.
