About The Position

This position is responsible for providing outreach to assigned EBR Library. The Library Outreach Case Manager provides linkage and referrals to community services in EBR, maintains knowledge of community resources, and serves as a liaison between assigned EBR library staff and patrons. The role involves documenting outreach contacts, referring individuals to services such as housing, job training, mental health, substance abuse, after-school programs, elderly services, health, and education. The position also requires adherence to payroll guidelines, reporting significant concerns to a supervisor, maintaining training requirements, and managing assigned program records. Projects and duties as assigned are also part of the role.

Requirements

Bachelor's degree in Social Work
One year of post graduate work experience
Excellent verbal & writing skills
Knowledge of various community resources
Valid driver's license
Auto insurance
Reliable transportation
Clean MVR record
Self-motivated
Ability to prioritize and multi-task

Responsibilities

Provide linkage & referrals to community services in EBR
Maintain sufficient knowledge of community resources
Provides liaison services between assigned EBR library staff & patrons
Document outreach contacts on appropriate forms
Refer individuals to eligible services in areas of housing, job training, mental health, substance abuse, after school programs, elderly services, health & education, etc.
Follow all guidelines & procedures related to payroll including but not limited to submitting contact logs by payroll deadline.
Report any significant concerns to Supervisor
Maintain training requirements
Maintain assigned program records.
Projects and duties as assigned
