Recognized leadership abilities and strong clinical background, who will provide assistance to the department manager in the continuing operation of the department, and facilitate the provision of professional services to meet the psychosocial needs of the patient through psychosocial assessments, brief treatment/counseling, crisis interventions, case consultation, patient education, resource management, and assistance with discharge planning. The Lead Social Worker will provide leadership to the other social workers in the department with guidance and oversight. The Lead Social Worker is an integral part of the interdisciplinary team, providing mental health expertise and community outreach/networking with community programs as needed. The Lead Social Worker works closely with social service staff to coordinate services for patients according to policies, procedures, and guidelines. This position requires providing care and/or service to the critically ill, rehabilitating newborn, and the geriatric patient population in a manner that demonstrates an understanding of the functional and/or developmental age of the individual served. This position requires the full understanding and active participation in fulfilling the Mission of San Gabriel Valley Medical Center. It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support San Gabriel Valley Medical Center’s strategic plan and the goals and direction of the Performance Improvement Plan (PIP).
