About The Position

The Lead Social Worker at CHOI Medical Group is responsible for promoting optimal clinical and resource outcomes by assessing, planning, implementing, coordinating, and monitoring interventions. This role involves developing comprehensive patient care plans and maintaining ongoing communication to enhance the quality of care through collaboration with the Interdisciplinary Care Team. The position coordinates healthcare services to ensure care is delivered at the appropriate level, optimizing clinical outcomes and patient satisfaction. The Social Worker/Program Coordinator assesses and addresses complex psychosocial and behavioral health issues related to population management and transition plans, demonstrating the necessary knowledge and skills for the assigned patient population. This role also works with the CHOI MG leadership team to align with strategic goals and OSF Key Results. Additionally, the Lead Social Worker provides daily support to frontline staff and entity leadership, triaging barriers and concerns. The lead serves as a subject matter expert, onboarding new employees, mentoring, and acting as a resource for all CHOI MG Mission Partners/Social Workers/Program Coordinators.

Requirements

  • Master’s degree in Social Work from a school or program accredited by the Council on Social Work Education.
  • 3 years of experience caring for patients with progressively increasing responsibility.
  • Broad knowledge of chronic illness and medical and community resources.
  • Positive, empathetic approach and ability to communicate effectively to patients/families with a variety of health issues, age groups, and socioeconomic backgrounds.
  • Substantial interpersonal skills to develop and maintain cooperative working relationships with personnel from other departments.
  • Working knowledge of a clinical EMR system and Microsoft Office applications.
  • Ability to work independently and efficiently with excellent time management skills.
  • Flexible work schedule – weekend/holiday/on call rotation.
  • Ability to successfully use independent judgment and make decisions with little supervision.
  • Illinois or Michigan state licensing and certification requirements or jurisdiction(s) in which she or he practices.
  • A Social Worker with an MSW who has not obtained their license will be required to pass the examination for the practice of social work and obtain applicable license as awarded by the Department of Professional Regulation within the employee’s designated state (Illinois/Michigan) of practice.
  • Within 1 year of completing specified supervised clinical hours, the employee will be required to pass the examination for the practice of social work and obtain applicable license as awarded by the Department of Professional Regulation within the employee’s designated state (Illinois/Michigan) of practice.
  • CPR Certification within 90 days of hire.

Nice To Haves

  • Chronic, complex illnesses adult and/or pediatric care management experience.
  • Care Management Certification as appropriate for adult or pediatrics.
