About The Position
Agawam West Rehab & Nursing is seeking a licensed social worker (MSW or BSW required) to join our skilled nursing and rehabilitation team in a full-time leadership role focused on admissions, discharge planning, resident support, and care coordination. This position offers the opportunity to work closely with residents, families, nursing, rehabilitation, and facility leadership to help ensure successful care transitions and positive resident outcomes.
Requirements
- Active social work license required ( LCSW, LICSW, or LSW )
- MSW or BSW required
- Skilled nursing, rehabilitation, or long-term care experience required
- Strong communication and organizational skills
Nice To Haves
- Knowledge of Medicaid and discharge planning preferred
Responsibilities
- Coordinate admissions and discharge planning processes
- Complete psychosocial assessments including BIMS and PHQ-9
- Support residents and families throughout their stay
- Participate in interdisciplinary care planning meetings
- Collaborate with nursing, therapy, and administration teams
- Maintain documentation and compliance within PointClickCare
- Assist with regulatory readiness and resident advocacy
