About The Position

As a member of the interdisciplinary team, the Social Worker provides services to respond to the emotional needs of residents and their families. This employee also assists with resident admissions and the referral process. The Social Worker is responsible for providing and arranging social work services as indicated by resident/family needs, transfers, and changes in level of assignment. They must possess a working knowledge and ability to apply professional standards of social work practice, demonstrate assessment skills sufficient to evaluate residents’ behavior and psychosocial needs, and interpret resident behavior to staff. The Social Worker is also responsible for completing the RAI process, coordinating and facilitating family meetings, implementing social service interventions, and tracking PASARR and OBRA screenings. They must carry out all duties in accordance with the facility mission and philosophy, respecting resident rights, dignity, and confidentiality. Ensuring the quality and appropriateness of resident care meets or exceeds company and industry standards, and that all services are in compliance with state, federal, legal, regulatory, accreditation, and reimbursement guidelines is also a key responsibility. Maintaining effective relationships and open communication with residents, families, and staff is essential.

Requirements

  • Minimum of bachelor’s degree in Social Work.
  • One year supervised social work experience working directly with individuals in a long-term or sub-acute care rehabilitation facility required.
  • Currently licensed as Social Worker in State of Michigan.
  • Knowledge of regulatory standards and compliance requirements.
  • Working knowledge of personal computer and software applications used in job functions (word processing, graphics, databases, spreadsheets, etc.).
  • Strong organizational, analytical, oral communication, and written communication skills.
  • Working knowledge and ability to apply professional standards of social work practice in job situations.
  • Current knowledge of state and federal laws and regulations that apply to the practice of social work in long-term care.
  • Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations.
  • Ability to communicate orally and through written reports and other documents relating to patient, staff, and others.
  • Ability to effectively present information and respond to questions from groups of executives, managers, clients, customers, and the public.
  • Ability to define and solve problems, collect data, establish facts, and draw valid conclusions.
  • Ability to interpret an extensive variety of technical instructions in statistical or diagram form and deal with several abstract and concrete variables.

Responsibilities

  • Meet with the resident and family to review policies on advanced directives, resident rights, and discrimination as part of the admission process.
  • Assist in completing admissions paperwork for newly admitted residents and families.
  • Conduct and document a social services evaluation, including identification of resident problems/needs.
  • Provide/arrange for social work services as indicated by resident/family needs, transfers, and changes of level assignment.
  • Establish criteria to assure that services provided meet established standards of quality.
  • Establish satisfactory interpersonal skills to interact with residents, families, community, and interdisciplinary staff.
  • Demonstrate assessment skills sufficient to evaluate residents’ behavior, data collected to evaluate psychosocial needs, risk factors for psychosocial deterioration, and residents’ responses to interventions.
  • Interpret and explain resident behavior to staff in a way that fosters understanding, facilitates treatment and respect for resident rights, and minimizes the use of chemical and physical restraints.
  • Complete the RAI process in a timely manner and maintain/update care plans as needed to meet resident needs.
  • Assist to coordinate family meetings and facilitate such meetings to ensure residents and families are kept informed of condition changes and interventions.
  • Implement social service interventions that achieve treatment goals, address resident needs, and link social supports, physical care, and physical environment to enhance quality of life.
  • Track PASARR screenings, OBRA screenings, and other related mental health screenings to assist the resident in maintaining good financial status and nursing home eligibility.
  • Express self adequately in written and/or oral communication and communicate effectively in an interdisciplinary care setting with residents, families, and staff members.
  • Carry out all duties in accord with the facility mission and philosophy.
  • Demonstrate knowledge of, and respect for, the rights, dignity, and individuality of each resident in all interactions.
  • Appreciate the importance of maintaining confidentiality of resident and family information.
  • Demonstrate honesty and integrity at all times in the care and use of resident and facility property.
  • Ensure that the quality and appropriateness of resident care meets or exceeds company and industry standards, and that all services are in compliance with state, federal, legal, regulatory, accreditation, and reimbursement guidelines.

  • Attend in-service and educational programs; attend continuing education required for maintenance certification.
