About The Position

The Lead Case Manager will provide case management services for a variety of students with complex health, functional, and social needs. The Lead Case Manager, will be required to serve as the point of contact to provide support and training to new case management staff, facilitate trainings on behalf of Amity Just in Reach/ODR projects. Additionally, this position will coordinate, develop and oversee specific administrative functions of the program.

Requirements

  • Proficiency with MS Office Suit, i.e., Word, Excel, PowerPoint, ect.
  • Understanding of causes, symptoms and effects of substance abuse; appreciation for the behavior, psychological, physical and social effects of drugs and drug abuse.
  • Excellent oral and written skills.
  • Ability to solve practical problems and carry out responsibilities under general supervision.
  • Ability to organize workload for effective implementation.
  • Ability to write and present information in one-on-one and small group situations.
  • Ability to interact effectively at all levels and across diverse cultures.
  • Possess excellent oral and written skills.
  • Ability to interact with multi-disciplinary groups of professionals and correctional officials.

Responsibilities

  • Support and work in collaboration with clients and case managers on the development of individual housing plans to help clients reach their goals.
  • Support onboarding and provide training and guidance to new ICMS case managers.
  • Support advocates with all phases of participant services including screening, intake and enrollment, case management, and exit and follow-up processes.
  • Interview and assess participant need through interviews with participants referred by the Office of Diversion and Reentry.
  • Verify and document the eligibility of participants.
  • Develop a service with the participant to identify and address service needs and goals.
  • Assist with public benefits advocacy.
  • Work with other faculty to assist with placement and case management upon release.
  • Collaborate with other area agencies and partners involved with the participant to ensure a coordinated effort to best serve the participant.
  • Attend inter-agency case coordinated meetings, coalitions, regional advocacy groups, and boards as appropriate in benefiting the participants served and support community cooperation in addressing areas of need.
  • Maintained concise and comprehensive case records on all participants seeking services.
  • Provide accurate daily and monthly statistical information as per funding source guidelines.
  • Coordinate trips for participants to housing and preparation appointments.
  • Will be required to visit county jails.
