Catholic Charities Bureau, Inc., Jacksonville Regional Office is seeking a Intensive Case Manager for the Preferred Communities Refugee Program This is a full-time, 40 hour a week (Monday to Friday, 8:30 am to 5:00 pm), non-exempt position located at 6 East Bay Street, Jacksonville, FL. The hiring range for this position is $22 to $24 an hour. Job Summary The PC-ICM Case Manager provides intensive case management services to eligible clients under the PC-ICM Program. The goal is to help clients achieve long-term self-sufficiency by addressing complex needs through a structured case management approach. This role plays a vital part in helping vulnerable refugee and ORR-eligible populations achieve self-sufficiency, navigate complex systems, and build successful lives in the U.S.

  • Client Intake & Enrollment Conduct client intake and pre-enrollment assessments.
  • Determine eligibility under the Office of Refugee Resettlement (ORR) guidelines.
  • Collect all required documentation and client signatures.
  • Provide program orientation to new clients.
  • Complete initial assessment within 7 days of enrollment.
  • Case Management & Planning Develop and implement customized self-sufficiency plans based on each client’s needs.
  • Conduct comprehensive assessments at 90, 180, 270, and 360-day intervals.
  • Manage a caseload of PC-ICM clients, maintaining regular contact.
  • Monitor progress toward goals and update case plans as needed.
  • Provide individualized support including: Life skills training (budgeting, housing, employment, legal, etc.) Scheduling appointments Making appropriate referrals Connecting clients to community resources Arranges for interpreter and/or accompaniment, as needed. Provides transportation to clients, as needed.
  • Program Compliance & Documentation Accurately document all services provided, case notes, and client communications in a timely manner.
  • Ensure compliance with program policies and procedures.
  • Complete timely and thorough case closures upon service completion.
  • Assist in the compilation of required reports for grants.
  • Track, manage, and report quarterly Performance Quality Improvement (PQI) data to support agency accreditation under the Council on Accreditation (COA) standards.
  • Monthly review case records and service plans to evaluate effectiveness and appropriateness of services delivered, and for process improvement.
  • Completes corrective action regarding deficiencies on the CRR compliance checklist as needed.
  • Collaboration Coordinate with ESOL (English for Speakers of Other Languages) program staff to support client language acquisition and integration.
  • Work collaboratively with internal staff and external partners to ensure delivery of core services and achievement of grant outcomes.
  • Keep supervisor informed of progress, problem areas, need for additional resources, successful accomplishment of milestones, and matters that may reflect favorably or unfavorably on the agency.
  • Maintain standards of ethical and moral conduct established by the Diocese of St. Augustine and Catholic Charities Bureau.
  • Consistently demonstrate a positive, friendly, respectful, and caring attitude with staff, clients, volunteers, and visitors, projecting a welcoming atmosphere to all.
  • Organize and prioritize work, be proactive, take initiative, resolve complex problems, follow through, and simultaneously manage multiple priorities.
  • Adhere to agency standards involving the handling of highly confidential and sensitive information.
  • Display sensitivity to the served population’s cultural and socioeconomic characteristics.
  • Attend all agency and team meetings as required.
  • Participate in in-service activities, training, continuing education, conferences and workshops for professional development and compliance with the grants and agency policies.
  • Keep detailed records per agency and department guidance.
  • Comply with regulatory, statutory, contractual, grant and agency standards for timeliness, completeness, and accuracy when preparing and maintaining paperwork and required reports.
  • Performs other duties as assigned.
  • Knowledge of all program guidelines and requirements.
  • Ability to report to work on time, follow directions from supervisor, and accept constructive feedback.
  • Exceptional interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  • Ability to interact compassionately and respectfully with clients, visitors, staff and board members.
  • Strong typing and computer skills are required, including knowledge of and proficiency with Microsoft Word, Outlook, Excel, and other software programs.
  • Excellent organizational, documentation, and time management skills.
  • High level of personal initiative and ability to work with minimal oversight.
  • Ability to act calmly in crisis situations and to resolve conflicts in a constructive and effective manner.
  • Enthusiasm and the ability to thrive in an atmosphere of constant change.
  • Understand and follow agency policies and procedures.
  • Bachelor’s degree in Social Work, Human Services, or a related field.
  • Minimum of 3 years of case management experience, ideally with refugee or immigrant populations.
  • Clear a Level II background screening.
  • Clear a reference check (professional and personal).
  • Clear a local background check from the county in which you reside.
  • Clear a Motor Vehicles Records check.
  • Provide proof of and maintain a minimum personal auto insurance according to agency standards.
  • Provide a copy of all certifications prior to your first day of employment.
  • Clear an E-Verify check.
  • Valid Florida state driver’s license and access to reliable transportation.
  • Driving and Travel 10% travel is required for this job.
  • This position requires that you drive your personal automobile or vehicle on agency business on behalf of Catholic Charities Bureau, Inc. Therefore, you must be at least 21 years old and have a current valid Florida Driver’s License with no more than 5 points on your Florida driver’s license within the last three years and no DUI convictions. You are required to carry automobile liability insurance in the amount of at least $50,000 for each person, $100,000 for each accident or occurrence for bodily injury, and $25,000 for property damage. Additionally, if transporting children, employees must be at least 25 years old and have increased limits of 100/300/50.   • Familiarity with ORR programs and eligibility categories is strongly preferred.
  • Strong understanding of trauma-informed care and cultural humility.
  • Fully bilingual preferred in Arabic, Dari, French, Haitian Creole, Spanish, or Swahili.
