A Case Manager Assistant, under the general review of the Access/Crisis Supervisor, acts as an assistant to Case Managers providing Intensive Case Management services. This Case Manager Assistant is a member of the multidisciplinary Intensive Case Management team providing intensive, community-based mental health services. Case Manager Assistants, with guidance and oversight of the Access/Crisis Supervisor, are responsible for case coordination, linking, monitoring, and implementation of all aspects of individual consumer plans of service for adults with mental illness, who may have a co-occurring substance use disorder or developmental disability. The Case Manager Assistant works collaboratively with team members to support client recovery, stability, and independence through individualized, strengths-based interventions delivered primarily in community settings.
