About The Position

A Case Manager Assistant, under the general review of the Access/Crisis Supervisor, acts as an assistant to Case Managers providing Intensive Case Management services. This Case Manager Assistant is a member of the multidisciplinary Intensive Case Management team providing intensive, community-based mental health services. Case Manager Assistants, with guidance and oversight of the Access/Crisis Supervisor, are responsible for case coordination, linking, monitoring, and implementation of all aspects of individual consumer plans of service for adults with mental illness, who may have a co-occurring substance use disorder or developmental disability. The Case Manager Assistant works collaboratively with team members to support client recovery, stability, and independence through individualized, strengths-based interventions delivered primarily in community settings. Requirements

  • Must demonstrate sensitivity, knowledge and use of appropriate approaches, skills and techniques which reflect an understanding and awareness of trauma and social, cultural and economic diversity of the target population served. GIHN employees will actively support the goal of person-centered planning and integrated care. Will demonstrate competencies related to the complexities of the human condition and the manner in which physical health, mental health, substance use, and trauma are interrelated. Extensive knowledge of computer software, specifically Microsoft Office Suite. Must demonstrate high level of interpersonal skills to handle sensitive and confidential situations and documentation and the ability to maintain a high level of confidentiality. Must show attention to detail in composing, typing and proofing materials, establishing priorities and meeting deadlines. Must exhibit strong spelling, grammar and both oral and written communication skills.
  • High School Degree Required.
  • Associates degree preferred
  • Experience as a current or previous recipient of mental health services preferred.
  • Peer Support Certification preferred.

  • Three to five years of experience.

Responsibilities

  • Planning and/or facilitating planning using person-centered principles.
  • Developing an individual plan of service using the person-centered planning process.
  • Linking to, coordinating with, follow-up of, advocacy with, and/or monitoring of Specialty Services and Supports and other community services/supports, as required to meet the needs of the consumer receiving services.
  • Actively participates in Intensive Case Management team huddles.
  • Assistance with access to entitlements and/or legal representation.
  • Overseeing implementation of the individual plan of service, including supporting the consumer’s dreams, goals, and desires for optimizing independence; promoting recovery; and assisting in the development and maintenance of natural supports.
  • Coordination with the Medicaid Health Plan, Medicaid fee-for-service, or other health care providers.
  • May serve as a member of the emergency services crisis team, providing 24-hour crisis intervention for the residents of Gratiot County.
  • Maintains a high level of confidentiality and professionalism at all times.
  • Protects the security, confidentiality, and integrity of all protected health information in a manner that is consistent with HIPAA, MDHHS and GIHN policy and procedure.
  • Assures consumers’ rights are protected and recipient rights procedures are followed throughout GIHN and its provider network.
  • Attends and actively participates in GIHN required training as well as seeks external education opportunities for professional growth and development.
  • Abides by all applicable GIHN policies and procedures.
  • Maintains and completes detailed records in compliance with regulatory and professional standards, GIHN policy and third-party funding source requirements
  • Maintains a safe and clean work environment.
  • Performs work and other duties as assigned by the supervisor
