About The Position

The Intensive Case Manager is responsible for providing substance use screenings and assessments, mental health referrals, treatment planning, advocacy, escorts, and coordination of services for homeless clients who are suffering from co-occurring disorders (substance abuse and mental illness).

Requirements

Must be able to work independently and with teams.
Experience working with the adolescent/young adult population required.
Experience in individual and group behavioral counseling.
Ability and willingness to work flexible hours, weekends and holidays.
Computer literacy, including knowledge of basic software applications (Google suite).
Familiarity with the internet and email communications.
Enters data and produces reports using agency systems with accuracy and attention to detail.
MSW degree required, or current and active enrollment toward said degree; NYS license preferred (LMSW/LCSW).

Nice To Haves

Knowledge of psychosocial needs of LGBTQ/homeless population is helpful.

Responsibilities

Conduct substance abuse screenings and assessments and develop treatment plans for AFC clients.
Provide ongoing intensive case management services (crisis counseling, outreach, referrals, advocacy, escorts, and discharge planning) to clients with co-occurring disorders.
Perform evidence-based practices as assigned and written documentation of services provided.
Facilitate regular occurring support groups at the Drop-In Center.
Participate in weekly treatment planning meetings and provide knowledge and expertise about substance issues to direct care staff.
Some weekend and evening hours required.
Attend weekly staff meeting and supervision and perform other duties as assigned.
