About The Position

Under direction, provides case management services and support to clients. Completes assessments, monitors, coordinates, and links individuals to resources. Determines eligibility, makes referrals, and assists participants in accessing benefits and services. Works with participants to identify and reach goals. Enters client information into client tracking databases. Develops a plan of service based on assessment. The Lead Case Manager will be responsible for providing comprehensive support to older adults experiencing or at risk of abuse, neglect or exploitation. This role will ensure the safety, well-being, and independence of clients by coordinating services, advocating for their needs, and collaborating with community partners to support their overall well-being. This part-time position is funded through the Office on Violence Against Women (OVW), U.S. Department of Justice grant. Funding is authorized through September 30, 2028, with the possibility of renewal based on continued federal support.

Requirements

  • Bachelor’s Degree from an accredited college or university in Human Services, Social Work, Psychology, Gerontology, Criminal Justice or a directly related field
  • A minimum of two (2) years experience of direct service case management
  • Working with victims of abuse, neglect and financial exploitation
  • Possession of a valid Michigan driver’s license and operable, insured automobile for authorized travel
  • Knowledge of the dynamics of abuse and the needs of victims
  • Knowledge of community resources, human service agencies, programs and services for disadvantaged populations
  • Excellent written and oral communication skills
  • Public speaking and presentations to adult learners
  • Organizational and initiative skills, including mult-tasking and time management to meet deadlines
  • Using good professional judgment for the welfare and safety of clients
  • Various software and computer programs, such as Microsoft Suite, Outlook, OneDrive
  • Ability to develop and maintain effective and appropriate working relationships
  • Ability to work with multiple disciplines such as law enforcement, criminal justice, and social service providers
  • Ability to work with diverse populations
  • Ability to work in an environment which embraces the county’s Dignity Campaign
  • Ability to effectively speak, write and understand the English language
  • Ability to understand and carry out oral and/or written instructions
  • Ability to accurately organize and maintain paper documents and electronic files
  • Ability to maintain the confidentiality of information and professional boundaries
  • Ability to work independently
  • Ability to conduct oneself with tact and courtesy

Nice To Haves

  • Master’s Degree from an accredited college or university in Human Services, Social Work, Psychology, Gerontology, Criminal Justice or a directly related field
  • License of Social Work in the State of Michigan (LBSW, LMSW)
  • Effectively speak, write and understand a language other than English is preferred

Responsibilities

  • Provides case management and assists in accessing supportive services and benefits to promote quality of life for program participants.
  • Determines eligibility for potential program participants under the direction of program staff.
  • Conducts needs assessment with each participant; develops a plan of service and goals with participants.
  • Works with participants to identify and reach goals as indicated in the plan of service.
  • Records all required participant and program information in client tracking database; maintains records and documentation as required.
  • Attends and participates in public meetings and presentations to explain available programs.
  • Attends and participates in all staff meetings.
  • Participates in case consults.
  • Facilitates the coordination of services with community partners.
  • Maintains and updates knowledge through conferences, seminars, and in-service trainings as directed.
  • Operates standard office equipment, including a personal computer.
  • Operates an automobile to perform assigned job functions.
  • Performs related duties as assigned.
