About The Position

The Caring Place (@Miami Rescue Mission & Broward Outreach Centers) is a leading provider of services to the homeless and needy of South Florida. We are looking for like-minded, dynamic individuals to come and join our team. The Caring Place provides emergency shelter, residential programs, job training, education & computer literacy classes, healthcare, transitional housing, permanent housing, employment opportunities for homeless men, women, and children as well as programs for “at-risk” youth. We are currently seeking a FT Lead Case Manager at the Broward Outreach Center in Hollywood, FL.

Requirements

  • Requires a High School diploma or GED; Addictions Training and/or Certification and/or a Bachelor’s degree in social work preferred, Certified Addictions Professional (CAP) a plus
  • Must have at least three (3) years of demonstrated experience working with the homeless population
  • Must be able to compile timely monthly outcome reports per program guidelines and requirements, service point updates, and other reports as assigned
  • Must possess excellent communication skills and be the liaison between Emergency Services Case Managers and the Behavioral Health Manager
  • Must be able to exercise independent judgment within the general framework of approved case management techniques, existing laws, and within agency guidelines
  • Must have the ability to recognize symptoms of common mental, physical, co-occurring, alcohol / substance abuse addictions to make proper assessment for care plans, referrals, and linkage with excellent follow up skills
  • Must possess knowledge of local community agencies, programs and resources available for clients
  • Must be able to establish and maintain effective working relationships with clients and staff of diverse cultural and linguistic backgrounds regardless of race, religion, age, sex, disability, political affiliation and sexual orientation
  • Must have the ability to react quickly and calmly in an emergency
  • Must have computer literacy and experience with MS Office Suite (Word, Excel and Outlook)
  • Must be able to understand, speak, read and write English and carry out oral and written instructions

Responsibilities

  • Provides assistance to Emergency Services Case Managers team with professional demeanor and cooperativeness
  • Communicates effectively with the Behavioral Health Manager.
  • Participates in staff file peer reviews of care plans and client file, performs follow-up to determine quantity and quality of service provided client and status of client’s case
  • Attends staff meetings for case review and consultation on difficult or unusual cases
  • Collaborates with Behavioral Health to ensure cohesiveness and meets regularly for crisis assessment team meetings weekly
  • Compiles timely monthly outcome reports per program guidelines and requirements, service point, and other duties as needed for the Behavioral Health Manager
  • Conducts needs assessments with all clients assigned, verifies data collected in intake, and makes recommendations for services
  • Ensures that all data entry information and initial care plans, case notes, and follow ups are entered into service point database daily
  • Monitors all cases assigned, keeps files up to date and in accordance with established policies and procedures and funding requirements
  • Properly documents all outcome measures for reporting purposes and all data is entered within three (3) business days upon initial referral and/or discharge of the client
  • Prepares accurate incident reports and keeps manager informed within a timely manner
  • Establishes and maintains effective working relationships with clients, while maintaining appropriate boundaries
  • Ensures that all clients are treated with respect and dignity and maintains client confidentiality
  • Helps client to self-identify destructive attitudes and behavior patterns by increasing awareness of personal accountability
  • Secures the cooperation of social service agencies and local community organization to meet client needs; advocates for the development and/or maintenance of services for clients refers client and assists with applications to Food Stamp office, Department of Children & Families (DCF), Department of Motor Vehicles (DMV), Social Security Administration (SSA), Department of Health/Vital Statistics (DHS – birth certificates), etc.
  • Will lead and/or participate in devotions, prayers, or other ministry activities ex. outreach events
  • Must be in agreement with and sign The Caring Place Statement of Faith
  • Performs other related duties as assigned
