About The Position

PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Interim Housing Services team as the Lead Case Manager at Bridges at Kraemer site. The Lead Case Manager will assist clients in breaking the cycle of homelessness by supporting clients in various phases, from street to housing. The lead case manager is a seasoned social services professional who collaborates with the multidisciplinary team and the team/program supervisor to support the needs of the client. The lead case manager maintains a direct client caseload and takes on additional duties to support the operations of the program and the program manager.

Requirements

  • Employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Successfully complete a Tuberculosis Test
  • Successfully complete a Background Screening
  • Successfully complete a Drug Test
  • Reliable transportation
  • A valid driver’s license
  • Proof of insurance and ownership for personal vehicles used during work duties
  • The ability to qualify for PATH's insurance coverage
  • Applicants must be authorized to work for any employer in the U.S. We are unable to sponsor or take over sponsorship of an employment Visa.
  • One (1) year of experience in healthcare, case management, homeless services or working with vulnerable populations.

Responsibilities

  • In collaboration with the Quality Assurance Department, ensure programs are in compliance with contracts and partnership agreements.
  • Under the supervision of the program manager, function as a contact representative for program matters.
  • Under the supervision of the program manager, provide administrative oversight of the team, which may include coverage for the program manager.
  • Support with the completion of critical incident reports as required per PATH policies and/or the contract.
