About The Position

The Lead Caregiver is responsible for providing caring and personalized assistance for residents living in our community. This role involves leading, managing, and being accountable for a team of caregivers, ensuring they uphold the community's 1440 care standards. The Lead Caregiver also communicates effectively with residents' families and reports any changes in resident needs to the Wellness Leader. The position requires providing direct care, including assistance with bathing, grooming, dining, mobility, toileting, and medication reminders. Documentation and inputting information into company systems are also key responsibilities. The role may require weekend availability and on-call duties.

Requirements

  • High school diploma or GED preferred
  • Previous health care or geriatric experience preferred
  • CNA or MA with 2 years of related experience or training
  • CPR/First Aid certification strongly preferred
  • Ability to apply common sense understanding to carry out written/oral/visual instructions

Responsibilities

  • Lead, manage, and overall accountability for your team and their performance
  • Upholding to work schedule, being ready to start work on time, remaining on the job for the whole duration of shift, and limited use of time off
  • Understanding and leading our 1440 care standards so you and your team ensure that each resident and each family has their needs met
  • Communicating messages tactfully and timely with resident’s families and ensuring the message has been received and correctly understood
  • Communicating resident changes immediately; providing timely and detailed information to your Wellness Leader regarding any changes to a resident’s needs or care
  • Leads and provides 1440 care
  • Provide compassionate and competent care for residents by performing the following services: Bathing, showering and grooming assistance, Serve and collect food trays; assist residents with dining, Wheelchair and walking escorts, Toileting and incontinence care assistance, Meal, activity, and medication reminder assistance, Wake up and turn down service, Upkeep of resident rooms
  • Answers resident call signals
  • Medication assistance/medication administration
  • Maintains appropriate and complete documentation per company policies
  • Input information into Yardi/CareStream
  • Available to work weekends and/or on call as necessary as assigned by the Wellness Coordinator
  • Work toward continual improvement of the overall organization
  • Perform other duties as assigned
