About The Position

Touching Hearts at Home is seeking a high-impact Lead Caregiver (Rapid Response CNA) to join our award-winning home care team. This leadership role is designed for a Certified Nursing Assistant who thrives in a dynamic environment and is committed to providing elite senior care. As a Lead Caregiver, you will serve as a first responder for urgent staffing needs, a mentor for new hires, and a specialist in Dementia, Alzheimer’s, and Hospice care. Requirements

Active CNA License (Certified Nursing Assistant) in Massachusetts.
2+ years of experience in Home Care, Assisted Living, or Skilled Nursing Facilities (SNF).
Proven expertise in Dementia Care and End-of-Life/Hospice protocols.
Strong communication skills with the ability to train and lead a team.
Valid Driver’s License and a reliable, insured vehicle (must be willing to transport clients).
Must be available Monday through Saturday, 7:00 AM – 7:00 PM.
Comfortable working in homes with pets and/or smokers.

Responsibilities

Provide emergency shift coverage for urgent client needs (arrival within 90 minutes of notification).
Introduce new caregivers to families and provide on-site mentorship and training.
Start care for new clients to ensure a seamless transition to permanent staffing.
Execute high-level care for seniors, including Hospice support and Memory Care.
Review and provide feedback on client care plans to improve health outcomes.
Assist with Activities of Daily Living (ADLs), including bathing, dressing, and grooming.

Guaranteed Pay: Base guarantee of 30 hours per week (with a typical 40-hour work week).
Overtime Opportunities: Earn 1.5x pay for hours exceeding 40.
