About The Position

Under the supervision of the HSTEAM Manager, the Lead HSTeam Coordinator provides high-intensity behavioral health support and rehabilitative services for residents of Harvey West Studios. This role is dedicated to clinical stabilization, psychiatric symptom management, and independent living skills training. As a Lead, they provide functional oversight for behavioral health documentation and act as the clinical "Zone Lead" for HSTeam staff agency-wide.

Requirements

  • B.A. or B.S. in Social Work, Psychology, or a related field.
  • 3+ years in community-based mental health or social services.
  • Experience with high-acuity behavioral health populations and clinical documentation is required.
  • Valid First Aid/CPR and NVCI (Non-Violent Crisis Intervention) certification (or ability to obtain within 30 days of hire).
  • Strong knowledge of trauma-informed care and the "Housing First" model.
  • Proficiency in Electronic Health Record (EHR) systems.

Responsibilities

  • Provide individual and group support focused on managing psychiatric symptoms, distress tolerance, and emotional regulation within a permanent supportive housing environment.
  • Provide direct training and assistance with Activities of Daily Living (ADLs) to ensure residents can maintain their independent housing units safely and successfully.
  • Responsible for the development and maintenance of "Problem Lists" and care plans that meet current standards for Medical Necessity within the County EHR.
  • Act as the lead site responder for behavioral health crises, utilizing trauma-informed care and NVCI (Non-Violent Crisis Intervention) techniques to maintain site safety and prevent hospitalization.
  • Coordinate closely with the Harvey West RN and external providers to monitor medication adherence and effectiveness.
  • Assist residents in identifying and reporting side effects and coordinating medical appointments to ensure holistic health stability.
  • Provide technical guidance to HSTeam staff on writing high-quality progress notes that accurately reflect rehabilitative goals and clinical outcomes.
  • Reviews site-specific data to ensure the team is meeting the requirement of tracking 100% of behavioral health service units in the County EHR for revenue projection purposes.
  • Leads clinical orientation for new behavioral health hires at Harvey West, ensuring they understand the unique acuity and safety protocols of the site.
  • Represents the behavioral health team in coordination meetings with County Behavioral Health and Full-Service Partnership (FSP) clinicians.
  • Acts as the "Zone Lead" for behavioral health resources in Santa Cruz County, maintaining an updated directory of dual-diagnosis support groups, recovery resources, and specialized therapy providers.
