About The Position

The Lead Advanced Health Practitioner (AHP) for Specialty Services provides leadership, direction, and guidance to Advanced Health Practitioners while delivering comprehensive collaborative patient management under the direction of board certified/eligible physicians across multiple specialty departments including but not limited to Oncology, OB/GYN, Gastroenterology, Cardiology, Orthopedics, Neurology, Endocrinology, Pulmonology, and other specialty services. The Lead AHP implements and maintains AHP requirements both programmatic and competency specific to Specialty Services Programs. This position is clinically responsible and accountable for the application of the AHP process, providing clinical guidance to the team, research involvement, consultative services, inter and intradepartmental liaison, education, leadership, and patient care management across the healthcare continuum from initial consultation through complex specialty care management.

Requirements

  • Graduate of an accredited Advanced Health Practitioner program required.
  • Minimum 7 years of experience in specialty care settings required.
  • Proficiency in specialty-specific procedures and protocols across designated specialty services.
  • Demonstrated expertise in management of complex medical conditions.
  • Strong leadership and mentoring capabilities in specialty care environments.
  • Experience with electronic health records and specialty practice management systems.
  • Current, unrestricted applicable Advanced Health Practitioner license issued by the State of CA.
  • Current certification by the appropriate national certification body.
  • Maintenance of appropriate CME requirements.
  • Current BLS or ACLS certification as required for specialty.
  • Additional specialty certifications as appropriate to scope of practice.
  • Active DEA license.
  • Written prescription privilege protocols signed by all attending physicians.

Nice To Haves

  • Experience with clinical research and quality improvement initiatives preferred but not required.

Responsibilities

  • Support and provide coordination and direction to advanced health practitioners through proactive communication across multiple specialty departments.
  • Direct, educate, and mentor staff in developing sound clinical judgment to promote positive patient outcomes in specialty care settings.
  • Serve as a preceptor by mentoring AHPs in specialized care delivery, resource utilization, and time management.
  • Function as a clinical resource, interprofessional consultant, and patient advocate across specialty services.
  • Partner with department leadership to manage daily work schedules, patient load assignments, and AHP call schedules across multiple specialties.
  • Implement department strategies to optimize efficiency through timely documentation, patient flow management, and other specified strategies.
  • Manage resource allocation based on patient needs and staff capabilities across specialty departments.
  • Coordinate clinical rotations for student Advanced Health Practitioners in specialty settings.
  • Collaborate in annual evaluation processes with physicians and management across specialty services.
  • Develop and supervise program development for specialty AHP services.
  • Participate in medical group committees that determine clinical policy and practice standards. E.g., AHP Integration Committee.
  • Engage in hospital and/or medical group Performance Improvement Programs.
  • Lead training and onboarding of new AHPs in specialty services.
  • Participate in Best Practice Teams for specialty care initiatives.
  • Review and provide input on specialty services policies and procedures.
  • Facilitate communication and coordination between specialty departments and primary care.
  • Perform and document comprehensive specialty-focused history and physical examinations.
  • Provide expert consultation and follow-up care for complex medical conditions across specialty areas.
  • Collaborate with specialists in developing and implementing treatment plans.
  • Coordinate care between multiple specialty services and primary care providers.
  • Perform specialty-specific procedures as appropriate to scope of practice and training.
  • Order and interpret advanced diagnostic tests including specialized imaging and laboratory studies.
  • Provide patient education specific to complex specialty conditions and treatments.
  • Manage hospitalized patients requiring specialty care oversight.
  • Conduct daily rounds and assessments for specialty service patients.
  • Coordinate with hospitalist services and other inpatient teams.
  • Facilitate discharge planning for complex specialty patients.
  • Manage transitions of care between inpatient and outpatient specialty services.
  • Provide consultation services to other hospital departments as needed.
  • Manage ongoing specialty care for established patients.
  • Coordinate pre-operative and post-operative care for surgical specialties.
  • Provide disease management for complex chronic conditions.
  • Facilitate referrals to sub-specialty services when indicated.
  • Manage specialty clinic operations and patient flow.
  • Participate in clinical research initiatives across specialty services.
  • Collect and analyze data for quality improvement projects.
  • Implement evidence-based practice guidelines in specialty care.
  • Participate in specialty-specific registries and outcome tracking.
  • Support clinical trials and research protocols as appropriate.
  • Document patient encounters accurately and timely in electronic medical records.
  • Communicate effectively with patients, families, and multidisciplinary healthcare teams.
  • Participate in multidisciplinary rounds and case conferences.
  • Maintain current knowledge of evidence-based practices and specialty-specific guidelines.
  • Ensure continuity of care through effective communication and care coordination.
  • Provide telephone consultations and triage for specialty services.
