About The Position

The Lead Advanced Health Practitioner (AHP) for Primary Care and Urgent Care provides leadership, direction, and guidance to Advanced Health Practitioners while delivering comprehensive collaborative patient management under the direction of board certified/eligible physicians. The Lead AHP implements and maintains AHP requirements both programmatic and competency-specific to Primary Care and Urgent Care Programs. This position is clinically responsible and accountable for the application of the AHP process, providing clinical guidance to the team, research involvement, consultative services, inter and intradepartmental liaison, education, leadership, and patient care management across the healthcare continuum from preventative care through acute care management.

Requirements

  • Graduate of an accredited Advanced Health Practitioner program required.
  • Minimum 5 years of Primary Care and/or Urgent Care experience required.
  • Proficiency in common primary care and urgent care procedures.
  • Demonstrated expertise in management of acute and chronic medical conditions.
  • Strong leadership and mentoring capabilities.
  • Experience with electronic health records and practice management systems.
  • Current, unrestricted, applicable Advanced Health Practitioner license issued by the State of CA.
  • Current certification by the appropriate national certification body.
  • Maintenance of appropriate CME requirements.
  • Current BLS certification.
  • Active DEA license.
  • Written prescription privilege protocols signed by all attending physicians.

Responsibilities

  • Support and provide coordination and direction to advanced health practitioners and other staff through proactive assistance.
  • Direct, educate, and mentor staff in developing sound clinical judgment to promote positive patient outcomes.
  • Serve as a preceptor by mentoring AHPs in care delivery, resource utilization, and time management.
  • Function as a clinical resource, interprofessional consultant, and patient advocate.
  • Partner with department leadership to manage daily work schedules, patient load assignments, and AHP call schedules.
  • Implement department strategies to optimize efficiency through timely documentation, patient flow management, and other specified strategies.
  • Manage resource allocation based on patient needs and staff capabilities.
  • Coordinate clinical rotations for student Advanced Health Practitioners.
  • Collaborate in annual evaluation processes with physicians and management.
  • Develop and supervise program development.
  • Participate in medical group committees that determine clinical policy and practice standards.
  • Engage in medical group Performance Improvement Programs.
  • Lead training and onboarding of new AHPs.
  • Participate in Best Practice Teams.
  • Review and provide input on Primary Care and Urgent Care policies and procedures.
  • Perform and document comprehensive history and physical examinations.
  • Provide routine preventive care including health maintenance visits, immunizations, and screenings.
  • Manage chronic conditions such as diabetes, hypertension, hyperlipidemia, and other common medical conditions.
  • Coordinate care with specialists and other healthcare providers.
  • Perform minor procedures including suturing, joint injections, and skin biopsies.
  • Order and interpret diagnostic tests including laboratory work, imaging studies, and EKGs.
  • Provide patient education and counseling on lifestyle modifications and disease prevention.
  • Manage medication therapy and monitor for drug interactions and side effects.
  • Evaluate and treat acute illnesses and injuries.
  • Manage conditions such as upper respiratory infections, urinary tract infections, minor trauma, and acute exacerbations of chronic conditions.
  • Perform wound care, splinting, and other minor procedures.
  • Determine appropriate level of care and facilitate referrals or transfers when necessary.
  • Provide rapid assessment and treatment of time-sensitive conditions.
  • Coordinate with emergency services when indicated.
  • Document patient encounters accurately and timely in electronic medical records.
  • Communicate effectively with patients, families, and healthcare team members.
  • Participate in quality improvement initiatives and clinical research.
  • Maintain current knowledge of evidence-based practices and clinical guidelines.
  • Ensure continuity of care through effective communication and care coordination.
  • Provide telephone consultations and triage as needed.
