About The Position

Promote and deliver the USRC Kidney Care Options Program to nephrology offices, hospital case managers, community partners, and USRC staff. The role supports the Dallas/Fort Worth, Amarillo, Lubbock, and Clovis, New Mexico areas and is based in the Dallas/Fort Worth area. It builds and maintains clinical relationships, educates patients with advanced chronic kidney disease (CKD) and end-stage kidney disease (ESKD), coordinates care in preparation for dialysis or transitions between modalities, and supports home therapy growth. The role also tracks program metrics and maintains documentation and communication across teams. The educator promotes the program and develops relationships to increase referrals, optimal access starts, transplant rates, and program awareness. Under direct RN supervision, the educator provides Kidney Care Options education to patients and families, follows up with patients according to their CKD progression, and works with care teams on ongoing education and home therapy assessment. The role coordinates patient needs such as dialysis access placement, transplant referrals, insurance resources, and transitions to USRC facilities. It collaborates with clinical and business partners, participates in outreach and training, and reports program metrics to leadership.

Requirements

Bachelor's degree or equivalent work experience.
Five years of experience in a healthcare organization working with patients and/or physicians in a clinical setting.
Knowledge of nephrology, End-Stage Renal Disease (ESRD), dialysis treatment, or a related field.
Excellent customer service skills and ability to communicate effectively verbally and in writing.
Intermediate computer skills, including Microsoft Office (Word, Excel, PowerPoint, Outlook) and web-based tools.
Proficiency in USRC applications required within 90 days of hire.

Responsibilities

Promote the Kidney Care Options Program to nephrology offices, hospital case managers, and USRC staff to drive referrals, expand home therapy pipelines, increase optimal access starts for in-center patients, improve transplant rates, and raise program awareness.
Develop and maintain relationships with external and internal physicians and hospitals, and follow up with nephrologists regarding education provided to their patients.
Plan, schedule, and deliver group and individual Kidney Care Options classes for CKD Stage 4 and 5 patients and their families.
Under direct RN supervision, provide Kidney Care Options education to incident in-center patients within 90 days of admission.
Collaborate with in-center teams to provide routine home therapy assessment and ongoing education for established patients.
Follow up with educated patients at the prescribed cadence based on CKD progression to maintain engagement and support transitions to therapy.
Participate in USRC team member training and events to promote knowledge of the Kidney Care Options Program.
Monitor and track patients using the Kidney Care Options Tracker platform, communicate patient status to in-center staff, home staff, and physicians, and report metrics to leadership on an established cadence.
Coordinate patient care needs, including dialysis access placement, transplant referral initiation, insurance resource navigation, and transition planning to USRC facilities.
Collaborate with Value Based Care to support optimal starts of treatment modalities where applicable.
Partner with Administrators, Social Workers, Home Therapy Registered Nurses, Regional Vice Presidents, Business Development, and Home Therapy Leadership to identify and pursue home growth opportunities.
Lead or participate in strategic outreach events with physicians and community partners.
Maintain required documentation standards and complete prescribed KCO education sessions and physician-engagement touchpoints.
