About The Position

Vasona Creek Healthcare Center is a premier, Medicare-certified skilled nursing and short-term rehabilitation facility located in beautiful Los Gatos, CA. We are dedicated to delivering patient-centered, high-quality clinical care. We are currently seeking a motivated, detail-oriented Kaiser Case Manager LVN to join our collaborative interdisciplinary team. In this role, you will serve as the primary liaison for our Kaiser Permanente residents, ensuring seamless care coordination, utilization review, and comprehensive discharge planning.

Requirements

Active, unrestricted California Licensed Vocational Nurse (LVN) license.
Minimum of 1–2 years of nursing experience.
Strong organizational capacity, exceptional communication skills, computer literacy, and an ability to navigate complex medical files.
Familiarity with skilled nursing facility (SNF) regulations, Medicare guidelines, and insurance authorization processes.

Nice To Haves

Prior case management, utilization review, or managed care experience (specifically working with Kaiser systems) is highly preferred.

Responsibilities

Manage patient care plans for assigned Kaiser Permanente residents from admission through discharge.
Complete regular utilization management reviews using standardized criteria (e.g., InterQual) to optimize appropriate lengths of stay and resource allocation.
Act as the central point of contact between Kaiser health plans, facility physicians, therapy departments, nursing staff, and families.
Coordinate safe transition plans, arrange home healthcare/durable medical equipment (DME), and prevent unnecessary hospital readmissions.
Maintain precise, timely case management documentation in compliance with federal, state, and managed care contract guidelines.
