Requirements

Current California LVN license in good standing.
Strong assessment, communication, and organizational skills.
Ability to manage multiple priorities in a fast-paced healthcare environment.
Commitment to providing exceptional resident-centered care.

Nice To Haves

Case management, skilled nursing, or long-term care experience preferred.
Knowledge of Medicare, Medi-Cal, managed care, and discharge planning processes is a plus.

Responsibilities

Coordinate and oversee resident care plans from admission through discharge.
Collaborate with physicians, nursing staff, therapists, residents, and families to ensure continuity of care.
Monitor clinical progress and identify opportunities to improve patient outcomes.
Facilitate care conferences and interdisciplinary team meetings.
Assist with discharge planning and transitions of care.
Ensure compliance with federal, state, and facility regulations.
Maintain accurate clinical documentation and case management records.
Advocate for residents while promoting quality care and customer satisfaction.
