Requirements

• Bachelor of Science Degree in Nursing
• Three (3) years of clinical experience in GI or Pulmonary services.

Nice To Haves

• Previous experience working with culturally diverse populations.
• Strong working knowledge of healthcare systems and community resources.
• Able to establish strong working relationships within a healthcare system.
• Previous experience in project management or case management.
• Masters of Science Degree in Nursing.

Responsibilities

• Managing the care of identified patients with interventional GI and Pulmonary needs, in collaboration with a multidisciplinary team, to ensure access to high quality care throughout the continuum.
• Serving as a single point of contact for patients, caregivers, and referring physicians.
• Providing resources and assistance with accessing clinical and supportive care services offered by Northside Hospital and community partners.
• Acting as a nurse with advanced training and skill who has experience working with Interventional GI or pulmonary procedural patients.
• Willingness to work flexible hours.
