About The Position

The candidate will serve as a trusted partner to children, families, and colleagues by helping ensure support and resources are accessible, responsive, and thoughtfully coordinated. This role plays an important part in creating a seamless and compassionate experience by listening closely, documenting needs with care, and connecting families to the right support at the right time. Requirements

  • Bachelor’s degree or higher in social work, human services, human development, education, or a related field;
  • Two years of experience working with children in a group setting;
  • A minimum of two years of experience working with children and families, including those from diverse populations, utilizing a strengths-based approach;
  • Strong interpersonal skills and the ability to work collaboratively with colleagues across departments to effectively meet the needs of staff, children, and families;
  • Health screening and TB clearance, to be renewed every five years;
  • Successful completion of required background screening; associated costs are covered by the organization;
  • Ability to correctly and effectively communicate in written and oral English;

Nice To Haves

  • A California driver’s license, daily use of an automobile, and automobile liability insurance (preferred);
  • Command of Spanish, Cantonese, Mandarin, or Vietnamese (preferred).

Responsibilities

  • Supervising, training, and evaluating intervention support services coordinators;
  • Ensuring that all requests receive a response within one business day;
  • Ensuring all requests are prioritized according to urgency and immediate health and safety risks;
  • Assigning support requests to the appropriate department;
  • Ensuring that the intervention support services team acts as a first responder to concerns and needs shared by families (grievances, complaints, etc.) promptly, appropriately documenting and assigning all concerns;
  • Coordinating with multidisciplinary teams to ensure integrated care;
  • Ensuring that the requestor receives a call, assuring that the request was received and heard, while getting clarification or additional information;
  • Tracking all requests, needs, and concerns with timelines of all actions taken;
  • Facilitating monthly case management meetings with the teams involved to update, track, and close all requests, needs, and/or referrals;
  • Documenting and tracking internal case management assignments, as well as referrals sent to external agencies (i.e., school district, mental health therapist, nutritionist, regional centers, etc.);
  • Inputting data into the organization’s software platform for tracking and maintaining timelines and working with the data team to troubleshoot any issues;
  • Maintaining staff and client confidentiality when responding to and assigning requests;
  • Collaborating with the Family Engagement and Education Support Services teams to monitor and document family needs, goals, and outcomes;
  • Collaborating with the Center-based Education Programs, Inclusion, Health Services, Behavior Health and Wellness, and Family Engagement teams to address the needs of the children, families, and teachers and to ensure that all staff receive support and training for children with challenging behaviors and behavior health needs;
  • Working effectively with other team members;
  • Assisting managers, directors, the vice president of educational programs, and the chief program officer;
  • Attending training;
  • Performing other duties as assigned.
