The Intervention Specialist - Trauma provides immediate trauma-informed emotional support, advocacy, and resources to hospitalized patients following trauma. This includes connecting patients with the clinical team, assisting with discharge planning, leading support and psychoeducation groups, and conducting safety planning. The role involves completing assessments and facilitating warm-handoffs to outpatient providers for ongoing care. Responsibilities also extend to responding to trauma alerts, safety calls, and pre-escalation needs, collaborating with street outreach teams, and facilitating mediation to mitigate retaliation risks. The specialist engages in intensive community outreach, making follow-up calls, connecting patients to resources, and sending letters to discharged patients. They schedule patient appointments with the Trauma Recovery Center (TRC), other internal Advocate resources, or community partners to ensure continuity of care, requiring strong collaboration with other providers. The position involves conducting intakes and needs assessments, linking patients to appropriate resources, and maintaining accurate records of patient interactions and action plans. The specialist works closely with the hospital treatment team and community partners to ensure comprehensive treatment, discharge planning, and continuity of care.
