The Intervention Specialist - Trauma at Advocate Aurora Health Corporate provides immediate trauma-informed emotional support, advocacy, and resources to hospitalized patients following trauma. Key responsibilities include connecting patients with clinical teams, assisting with discharge planning, leading support and psychoeducation groups, and safety planning. The role also involves conducting assessments, facilitating warm-handoffs to outpatient providers, responding to trauma alerts and safety calls, and collaborating with street outreach teams to mitigate retaliation risks. Additionally, the specialist engages in intensive community outreach, follow-up calls, and scheduling appointments to ensure continuity of care for discharged patients.
