About The Position

This role consists of promoting access to and maintenance of housing for vulnerable individuals who have experienced or are at risk of homelessness. The objective is to offer sustainable support adapted to each person's reality to prevent a return to the streets, using a harm reduction approach. The intervener works directly in the living environment and collaborates with the community and partners to support residential stability and meet individual needs. This support can be part of various programs, including those targeting former combatants, mental health issues, co-occurring disorders, or specific populations.

Requirements

  • College diploma or bachelor's degree in a relevant field and 1 to 2 years of relevant work experience OR any relevant and equivalent combination of experience and training.
  • Knowledge of the network of community resources, health services, and social services and their functioning.
  • Empathy, active listening, and helping relationship skills.
  • Ability to assess and intervene in crisis, conflict, or emergency situations.
  • Ability to work in a team, collaboratively, and in partnership.
  • Good organizational skills.
  • Good oral and written communication skills.
  • Proficiency in French and English (oral and written) to support our users.
  • Proficiency in computer systems and word processing software.
  • Discretion, patience, integrity, autonomy, punctuality, and regular attendance.
  • Successfully pass the written test and interview required by a program, if applicable.

Nice To Haves

  • Training in crisis management, emergency situations, and/or first aid, an asset.

Responsibilities

  • Welcome users and ensure their integration into services.
  • Identify user needs to refer, accompany, and/or integrate them into appropriate services.
  • Conduct needs assessments and ensure the implementation of an intervention and/or housing integration plan.
  • Establish a trusting relationship to facilitate accompaniment and intervention in the daily lives of users.
  • Stimulate each user's intrinsic motivation and support them in their biopsychosocial endeavors (financial, physical and mental health, addiction, legal, citizenship...).
  • Offer personalized support by meeting users in their living environment, while ensuring the adequate maintenance of their environment.
  • Accompany and intervene with users to support them in maintaining and/or developing skills related to daily life activities.
  • Facilitate group meetings as needed.
  • Ensure adequate and professional completion of follow-up and file management tools, reports, and perform requested communications.
  • Provide a supportive framework and intervene professionally during crisis situations.
  • Ensure compliance with codes of conduct and provide necessary guidance for maintaining a safe and stable living environment.
  • Participate in the follow-up of stay fees and the implementation of the budget plan.
  • Ensure liaison and accompaniment, when necessary, with treatment teams and various external professionals and interveners for their participants.
  • Welcome and accompany partners from the social, health, educational, and other sectors.
  • Perform any other related tasks.
