About The Position

The Access and Diversion Qualified Mental Health Professional—Community Services (QMHP-CS) serves individuals presenting to the LifePath Systems Diversion Center. The role provides face-to-face crisis assessments, crisis intervention and follow-up, relapse prevention, intake assessments, recovery and safety planning, resource navigation, referrals, and connections to outpatient behavioral health services. It connects community members with LifePath Systems services and works to provide timely access to appropriate care while diverting individuals from emergency rooms, hospitals, and jails. The position is cross-trained with the Crisis Center to assess and support crisis resolution and stabilization and link individuals to services that reduce inpatient and law-enforcement interventions. Services are delivered using trauma-informed and person-centered approaches. The QMHP-CS works with LifePath Systems teams, community organizations, local police agencies, mental health professionals, fidelity managers, and leadership to identify needs, reduce barriers, coordinate resources, and improve outcomes. The role requires professional conduct, appropriate boundaries, independent and collaborative work, and calm, respectful assessment and de-escalation of behavioral health crises. It also requires compliance with Health and Human Services Commission requirements, Texas Administrative Code, contracts, grants, and LifePath Systems policies and procedures, as well as timely completion of training and proper care of center-issued equipment. The position is scheduled Monday through Friday, 3 p.m. to 11 p.m. Because the center operates 24/7, employees may be required to work center holidays when a holiday falls on their scheduled workday. Key performance expectations include documenting engagement and customer service in the electronic health record for at least half of time worked; signing at least 90% of service notes within two business days; completing and signing crisis documentation by the end of the shift; completing assigned follow-ups at 24 hours and at days 1–7, 30, 60, and 90; communicating a shift report at each shift’s end; and achieving at least 80% on full chart reviews following the Golden Thread. The position also attends monthly team meetings. Bilingual employees receiving a stipend must be available to translate for staff as needed and use approved language in service provision.
