About The Position

The Intensive Housing & Health Case Manager is a vital member of the Supportive Housing Program (SHP) team, delivering comprehensive, individualized support to participants using a client-centered, harm reduction, and trauma-informed approach. This role blends hands-on housing case management with health navigation, helping participants achieve and maintain stability in both their housing and healthcare. This position involves regular home visits, accompanying participants to medical and benefits appointments, and connecting them to community-based resources — all while ensuring full compliance with funder documentation requirements, including the Illinois Department of Human Services (IDHS) and Housing Opportunities for People with AIDS (HOPWA).

Requirements

  • High school diploma or equivalent
  • 3+ years of case management experience
  • Valid driver's license and access to a personal vehicle
  • Comfort navigating Chicago by personal vehicle and meeting participants in their homes and community settings
  • Knowledge of supportive housing models and systems
  • Proficiency with case management databases
  • Strong interpersonal and relationship-building skills
  • Exceptional attention to detail and follow-through
  • Ability to work independently and manage a varied caseload

Nice To Haves

  • Bachelor's degree in social work, public health, or a related field
  • Experience with crisis prevention, intervention, goal-setting, and resolution techniques

Responsibilities

  • Conduct monthly home visits to assess and support housing stability
  • Build trusting relationships with participants using a strengths-based, trauma-informed lens
  • Provide crisis prevention and intervention using harm reduction philosophies
  • Facilitate access to Emergency Financial Assistance (EFA) when housing stability is at risk
  • Serve as a liaison between participants and landlords or property managers
  • Coordinate move-ins, lease renewals, unit inspections, and eviction prevention efforts
  • Complete and submit housing leasing paperwork to AFC within required deadlines
  • Collaborate with the Behavioral Health Specialist to conduct psychosocial assessments within 90 days of intake, where appropriate
  • Develop individualized service plans in partnership with each participant
  • Connect participants to relevant services and community resources
  • Support participants in building life skills and engaging in meaningful daytime activities
  • Assist participants in enrolling in public benefit programs such as SNAP, SOAR, and employment readiness initiatives
  • Verify and support participant engagement in medical care
  • Obtain and track client-level health data, including viral load and medical visit dates
  • Refer and link participants to services that promote positive health outcomes, treatment adherence, and greater self-sufficiency
  • Monitor participant follow-through with medical appointments and treatment plans
  • Coordinate with primary care providers on participant medical care plans
  • Provide education and support around medication adherence, appointment management, and treatment-related issues
  • Conduct outreach to medical providers to gather required clinical data
  • Connect eligible participants with public and private programs including Medicaid, Medicare, Part D, and ADAP
  • Enter participant intake information into required databases (Caseworky, Provide, etc.) within designated timelines
  • Record case notes and all required data elements accurately and consistently
  • Complete reassessments per SHP policies and funder requirements, including IDHS
  • Process annual lease renewals and complete associated workflows in applicable databases
  • Maintain thorough and up-to-date participant files in compliance with program standards
