About The Position

Hamilton Health Center is seeking an Intensive Case Manager I to provide comprehensive case management, care coordination, and advocacy for individuals receiving recovery and supportive services. In this role, you will work closely with clients, healthcare providers, and community partners to coordinate services, reduce barriers to care, and support each client's recovery journey through individualized service planning and ongoing care coordination. This position is ideal for someone who is passionate about behavioral health, recovery services, community engagement, and making a meaningful impact in the lives of individuals experiencing complex health and social needs.

Requirements

  • Bachelor's degree in Human Services or a related field
  • Strong interpersonal, communication, and organizational skills
  • Basic computer and data entry proficiency
  • Fundamental understanding of healthcare services and medical terminology

Nice To Haves

  • CPR and/or First Aid certification
  • One (1) year of experience in a medical or physician office setting
  • Knowledge of Joint Commission standards
  • Bilingual in Spanish, Nepali, or French

Responsibilities

  • Provide comprehensive case management, care coordination, and advocacy for clients receiving recovery services
  • Develop, implement, and update individualized service plans to address client needs and monitor progress
  • Coordinate primary care and behavioral health services within the continuum of care
  • Assess client needs and coordinate holistic care plans
  • Maintain ongoing communication with clients, families, and support systems
  • Connect clients with housing, legal, transportation, and community resources
  • Document service coordination activities and maintain accurate records
  • Build collaborative relationships with healthcare providers, community agencies, and civic organizations
  • Participate in required meetings, trainings, outreach events, and continuing education
  • Maintain required certifications and complete annual training requirements
  • Ensure compliance with confidentiality standards and regulatory requirements
  • Perform other related duties as assigned.
