Integrative Primary Care RN - NPIG (Full-Time, Day)
About The Position
NorthBay Integrative Primary Care (IPC) believes that lasting health is created through sustainable lifestyle change, meaningful patient partnerships, and whole-person care. The Integrative Primary Care RN serves as a trusted partner within the Integrative Primary Care Program, working in close collaboration with the Program MD’s and an on-site interdisciplinary team — including RNs, dietitians, and movement, stress management, and sleep specialists, and medical acupuncturists — to offer a highly personalized, relationship-based care experience built around a smaller patient panel. The Integrative Primary Care Program measures success not only by the clinical care delivered, but by patients’ ability to feel informed, supported, and engaged as active partners in their own health. While not a concierge medicine practice, the Integrative Primary Care participants do pay a program fee, not covered by insurance. At NorthBay Health, the Program RN provides complex care management and individualized primary and preventive care. Guided by evidence-based Lifestyle Medicine principles, the role helps patients prevent, reverse, and manage chronic disease through sustainable behavior change. Within the integrative care model, the RN supports patients through individual coaching, shared medical appointments, group education, and coordination with the on-site interdisciplinary lifestyle and integrative medicine team, delivering an enhanced, concierge-like care experience. Working collaboratively with patients, providers, and the lifestyle medicine team, the RN serves as the direct liaison between the provider and patient, ensuring questions and concerns are addressed, directing annual lab and on site group learning engagement, coordinating access to NorthBay’s network of specialists, and empowering patient participants to achieve meaningful and sustainable lifestyle transformation. The ideal candidate personally embraces Lifestyle Medicine fundamentals and has a desire for continuous growth and development in the professional practice of nursing.
Requirements
- Graduated from an accredited nursing school or college.
- BSN is required (excludes those employees grandfathered).
- Completion of American College of Lifestyle Medicine course (7.5 hours) will be required within 30 days of start of the position.
- Current California state RN license required.
- AHA or equivalent BLS required.
- Minimum two (2) years RN experience.
- Phlebotomy experience required.
- Knowledge of: CMS, TJC and EMTALA standards and federal and state regulations.
- Knowledge of telephone-based clinical assessment techniques. Current NCQA and HEDIS quality standards, trends in health care, best practices, management tools, and familiarity with related community resources and literature.
- This position will ensure that the patient experience provided at all levels exceeds the expectations of physicians, patients and team members and is provided in a financially responsible manner. This position may also function as a consultant and educator responsible for supporting patient participation and outreach goals, developing educational materials, conducting new patient member orientation, managing projects and promoting awareness. Effective public speaking skills for patient education and program development required.
- Computing skills including experience with MS Office (Excel, Word, PowerPoint, etc.) and data using electronic medical records to check patient history and documentation. Skills to use secondary portals to support and fulfill program participant engagement.
- Ability to effectively communicate with all levels of patients, physicians, health care personnel, supervisory staff, and peers and elicit information needed to make brief evaluations.
- Demonstrates the exceptional ability to be organized and efficient in prioritizing and managing assignments with minimal oversight and direction.
- Demonstrates the willingness to research, learn, and obtain knowledge for the performance of the position.
- Demonstrates a courteous, professional demeanor at all times either working individually or in a team environment working collaboratively in an effective manner maintaining positive relationships with peers and supervisors alike.
- Utilizes critical thinking and applies sound clinical judgment and assessment skills for decision making.
- Must be able to function in a multi-task environment.

Responsibilities
- Collaborate as part of a 1:1 MD/RN care team within the Integrative Primary Care Program.
- Manage complex care for an outpatient population with chronic health conditions.
- Demonstrate a high level of service orientation in an outpatient program where patients pay a fee for access and participation.
- Independently carry out clinical RN duties in an outpatient setting without the support of a medical assistant, phlebotomist, or administrative staff including: Comprehensive patient assessment, Take vitals, maintains exam rooms, Phlebotomy-ordering/fulling/preparing personalized lab specimens, Counsel patients, Coordinates referrals and follow up, Schedules testing, Fulfills EHR documentation requirements, Documents in other portals as program components require, Maintaining patient correspondence and communication in a timely and efficient manner.
- Collaborates closely with physicians and the interdisciplinary care team to coordinate individualized patient care plans and reinforce lifestyle interventions.
- Facilitates individual visits and educational group classes supporting lifestyle medicine principles.
- Assists in developing educational materials, wellness resources, and patient engagement tools that reflect current evidence and NorthBay Health branding standards.
- Supports lifestyle medicine, wellness, and community outreach initiatives that promote healthy living and disease prevention.
- Participates in quality improvement initiatives, program development, and outcome measurement to continually enhance patient experience and clinical effectiveness.
- Maintains current knowledge of emerging research in lifestyle medicine, integrative medicine and chronic disease prevention.
- Demonstrates exceptional communication skills while building trusting, compassionate relationships with patients, families, providers, and colleagues.
- Serves as an advocate for whole-person, patient-centered care that empowers individuals to become active participants in their health journey.
- Adheres to policies, industry standards, best practices.
- Promotes a quality, high touch, high service patient experience at all times.
