The Per Diem Integrative Health & Lifestyle Specialist (FGP) plays a crucial role in translating precision and lifestyle medicine into practical, daily actions within the Griffin Healthspan & Vitality Center. This position combines behavior-change expertise with care-journey coordination, assisting patients in implementing personalized care plans and lifestyle directives recommended by the clinical team. The Health Coach focuses on key areas such as nutrition, movement, sleep, stress management, and recovery, empowering patients to build sustainable habits and enhance healthspan markers. The Specialist collaborates with a multidisciplinary team, including physicians, NPs/APPs, acupuncturists, massage therapists, yoga/mind-body practitioners, dietitians, and mental health providers, to ensure coordinated, patient-centered care aligned with evidence-based integrative and lifestyle medicine. This is a non-clinical role focused on enhancing self-management, adherence, and patient activation as a supportive resource to the clinical team. The Specialist supports adult patients with or at risk for cardiometabolic and related chronic conditions in making sustainable lifestyle changes across the pillars of lifestyle medicine.

Requirements

  • Bachelor's degree in a health-related field (e.g., health education, public health, nutrition, exercise science, psychology, nursing, or related discipline).
  • Nationally recognized coaching, medical, fitness, or allied health certification (e.g., NBHWC health coach, RN, RD, PT, exercise physiologist, LMSW, or similar).
  • Minimum of 3 years of client-facing coaching or applied health experience in a clinical, wellness, or academic medical setting.
  • Demonstrated proficiency in motivational interviewing and patient-centered counseling.
  • Proficiency with EHR systems and comfort with telehealth and digital health platforms.
  • Strong organizational and multitasking skills with attention to detail.
  • Excellent communication and interpersonal abilities; commitment to precision, discretion, and client care excellence.
  • Qualified candidates must be able to effectively communicate with all levels of the organization.

Nice To Haves

  • Formal training and/or certification in Integrative Health, Lifestyle Medicine, Functional Medicine, or related disciplines.
  • Dual certification or cross-training in complementary fields (e.g., health coaching plus fitness, mind-body, or holistic modalities).
  • Certified Personal Trainer (CPT) or accredited health fitness certification (e.g., ACSM, NASM, ACE or equivalent).
  • Yoga therapy certification or advanced training in yoga for therapeutic or clinical populations.
  • Demonstrated experience with mind-body therapies (e.g., meditation, breathwork, mindfulness-based stress reduction, guided imagery, or similar).
  • Holistic or complementary health certifications such as Reiki, reflexology, integrative nutrition, or comparable modalities.
  • Experience facilitating group classes or group-based behavior change programs.
  • Experience working in integrative medicine, lifestyle medicine, or academic wellness/healthspan centers.
  • Experience with outcomes tracking, registries, or quality improvement projects in a clinical or wellness setting.

Responsibilities

  • Translate physician and APP recommendations into actionable daily strategies that fit the patients lifestyle, values, and cultural context.
  • Conduct structured health and wellness assessments (e.g., movement, nutrition, sleep, stress) to inform individualized coaching plans.
  • Partner with patients to set and refine personalized goals across movement, nutrition, sleep, stress management, and recovery using motivational interviewing and evidence-based behavior-change methods.
  • Conduct individual and group coaching sessions focused on adherence, accountability, mindset transformation, and self-efficacy.
  • Co-lead and assist in the development of curricula for group visits and/or shared medical appointments focused on lifestyle behavior change and patient education.
  • Identify barriers to progress (e.g., social determinants of health, readiness for change, environmental challenges) and develop tailored solutions to support consistency and long-term success.
  • Provide evidence-based education and resources to build patient understanding of integrative and lifestyle medicine and to reinforce the clinical care plan.
  • Collaborate with clinical and nutrition staff to develop and disseminate culturally appropriate educational materials that meet the needs of a diverse patient population.
  • Execute milestone touchpoints (e.g., new patient onboarding, key clinical milestones, anniversaries, pre-/post-travel) to maintain engagement and continuity.
  • Own end-to-end onboarding for Healthspan & Vitality Center patients, including profile setup, forms, EHR access, orientation to clinical and lifestyle services, and explanation of care pathways.
  • Design an individualized journey and roadmap for each patient in collaboration with the clinical team, including visit cadence, labs, imaging, referrals, and lifestyle programs.
  • Coordinate calendars across physicians, labs, imaging, internal and external partners; manage no-show and reschedule workflows with appropriate follow-up and documentation.
  • Triage inbound clinical and administrative questions from patients; escalate to the physician/APP or other team members per established protocols.
  • Oversee patient flow through group-based programs, including assigning patients to cohorts, helping track attendance, and partnering with program staff to ensure coordinated, continuous care.
  • Maintain complete, accurate records across the EMR and any supporting CRM or tracking tools; create structured visit notes and manage goals within the EHR in accordance with NYU Langone Health standards.
  • Produce after-visit summaries, co-own follow-through of lifestyle directives, and perform readiness checks for new interventions.
  • Track all clinical directives (labs, referrals, imaging, lifestyle tasks) to closure; notify stakeholders of incomplete items and barriers.
  • Upload and organize results such as body composition data, movement tests, biomarkers, and specialty labs.
  • Support design and iteration of LLM-assisted documentation and pre-visit preparation workflows (where applicable).
  • Onboard and troubleshoot continuous glucose monitoring (CGM) and wearables; ensure data ingestion into EHR dashboards and relevant platforms.
  • Educate patients on digital tools, portals, and EHR navigation to enhance engagement and transparency.
  • Track progress, celebrate achievements, and adjust plans based on outcomes, biometrics, and patient-reported data.
  • Present patient progress, risk flags, and adherence patterns during clinic rounds and team huddles.
  • Notice and correct errors in orders, records, and scheduling workflows; support ongoing process improvement.
