Integrated Nurse Case Manager
About The Position
Sentara Health Plans is hiring a remote Integrated Nurse Case Manager in the Richmond, Virginia area. The role is primarily work from home and requires travel to assigned nursing facilities within the Richmond area. Hours of operation are Monday through Friday, 8:00 a.m. to 5:00 p.m. The Integrated Nurse Case Manager performs casework and provides case management services to members in nursing facilities, including patients, families, and designated caregivers. The position develops, participates in, and monitors multidisciplinary collaboration of services where appropriate. It also assists adult patients and their families with personal and environmental difficulties related to medical conditions, including terminal illnesses.
Requirements
- Registered Nurse certification is required.
- Three years of nursing experience is required.
- Applicants must be willing to travel within the Richmond area.
Nice To Haves
- Bachelor's degree preferred.
- Managed care experience preferred.
- Discharge planning experience preferred.
- Case management or care coordination experience preferred.
- Experience with community health resources preferred.
- Experience initiating care plans and conducting subjective and objective assessments preferred.
Responsibilities
- Perform casework and provide case management services to members in nursing facilities.
- Work with patients, families, and designated caregivers.

- Assist adult patients and their families with personal and environmental difficulties associated with medical conditions, including terminal illnesses.
