As an Integrated Care Specialist, Licensed Social Worker, you’ll play a meaningful role in providing person-centered, trauma-informed support and coordination of services for individuals with intellectual and developmental disabilities (IDD), including those with co-occurring behavioral health needs (e.g., ASD, mental health conditions). This role focuses on improving functioning across life domains including behavioral health, adaptive skills, communication, and community integration.

Requirements

  • Bachelor’s degree in social work
  • Experience working with people with mental health conditions.
  • Ability to obtain and maintain First Aid, CPR - adult, child, and infant. (Training will be provided, if not already certified).
  • Personal vehicle to transport clients as needed.
  • Able to pass training to become certified to provide physical intervention as needed
  • Valid driver’s license and insurable driving record, if applicable.
  • Valid car insurance
  • Licensed through the State of Ohio as a Licensed Social Worker (LSW)

Nice To Haves

  • Experience working with people with autism spectrum disorders and developmental disabilities preferred.
