About The Position

I Am Boundless is seeking an Integrated Care Specialist - LSW to join their non-profit organization that specializes in assisting individuals with Intellectual and Developmental Disabilities (IDD). The mission is to build a world that realizes the boundless potential of all people. This role provides person-centered, trauma-informed support and coordination of services for individuals with IDD, including those with co-occurring behavioral health needs. The focus is on improving functioning across life domains such as behavioral health, adaptive skills, communication, and community integration. Services can be provided in person or via telehealth, in various settings including homes, schools, and other community locations. The role is expected to successfully treat moderate acuity cases and may require more supervision for high acuity cases. The position may be filled by unlicensed professionals with relevant experience or licensed clinicians, with responsibilities adjusted based on scope of practice. The role receives regular clinical supervision and case review for training, support, and professional development.

Requirements

  • Bachelor’s degree in social work
  • Licensed through the State of Ohio as a Licensed Social Worker (LSW)
  • Experience working with people with mental health conditions.
  • Ability to obtain and maintain First Aid, CPR - adult, child, and infant.
  • Personal vehicle to transport clients as needed.
  • Able to pass training to become certified to provide physical intervention as needed.
  • Valid driver’s license and insurable driving record.
  • Valid car insurance.
  • Valid Ohio Driver’s License with Ohio Bureau of Motor Vehicles - No more than 5 points on driving record.
  • Current High-Fidelity Wrap-Around and CANS Assessor training or the ability to complete such within 90 days of hire.

Responsibilities

  • Providing person-centered, trauma-informed support and coordination of services for individuals with intellectual and developmental disabilities (IDD), including those with co-occurring behavioral health needs.
  • Improving functioning across life domains including behavioral health, adaptive skills, communication, and community integration.
  • Providing individual, group, or family services in an assigned treatment program, office, or community-based settings (including homes, schools, and other community settings).
  • Receiving regular clinical supervision and case review.
  • Providing services in person or via telehealth, dependent upon client preference and clinical appropriateness.
  • Obtaining and maintaining First Aid, CPR - adult, child, and infant certification.
  • Becoming certified to provide physical intervention as needed.
  • Transporting clients as needed.
  • Covering rotation for on-call after-hours coverage to support crisis needs.
