About The Position

The Intake Worker (Social Services) is responsible for supporting the initial client engagement process within Street Haven's addictions, mental health and outreach services team. This role involves supporting the Team Lead and Intake and Assessment Case Manager with preliminary assessments, gathering client information, coordinating appointments with mental health professionals, and maintaining accurate and confidential records. The position requires sensitivity, active listening skills, and the ability to work compassionately with individuals experiencing mental health challenges while ensuring a welcoming and supportive first point of contact for clients seeking services.

Responsibilities

• Assist with preliminary intake assessments with new clients, gathering essential demographic, medical history, and presenting concern information in a compassionate and non-judgmental manner.
• Supporting with scheduling and coordinating appointments with appropriate mental health professionals, including counselors, psychiatrists, and social workers, based on client needs and urgency.
• Maintain accurate, confidential, and up-to-date client records in compliance with privacy legislation and organizational policies.
• Assist with communicating available mental health services, programs, and support options to clients, ensuring they understand the intake process and next steps.
• Assist with following-up with clients who have missed appointments or are awaiting services, providing encouragement and addressing barriers to engagement.
• Assist with preparing intake summaries and note-taking to ensure proper documentation.
• Contribute to a welcoming and trauma-informed environment by greeting clients warmly, managing waiting areas.
• Other duties as assigned.
