About The Position

The Intake Specialist role provides frontline intake services to members joining New Freedom’s Intensive Outpatient Program. The Intake Specialist is one of the initial points of contact to ensure members have positive and successful entry into our facility. This position is accountable for reviewing eligibility and program expectations prior to members meeting with their assigned clinical workers. This position requires a thorough understanding of the company’s philosophy of treatment, admission processes, and program criteria. A basic knowledge of substance abuse and dual diagnosis is also required. This role is responsible for ensuring that clinical and administrative intake services are provided for clients in partnership with the clinical team and other cross-functional departments.

Responsibilities

  • Initiates all lobby packet documentation, including consent and admission forms.
  • Conducts comprehensive intake assessments, accurately identifying and documenting appropriate diagnoses.
  • Develops individualized treatment plans aligned with goals identified during the assessment process.
  • Provides thorough program orientation, ensuring member understanding of informed consent, program policies, rights, responsibilities, confidentiality, treatment expectations, and all required state and federal regulations.
  • Reviews program options, grievance and appeals processes, advance directives, comorbidity resources, after hours access, code of ethics, and all policies related to safety, substances, and program participation; supplies and explains the member handbook.
  • Ensures all new admissions and readmissions receive full orientation on the first day when possible, and always within seven working days of admission.
  • Completes warm hand offs to clinical and cross functional support teams once all intake tasks are finalized.
  • Meets established KPIs, actively participates in required clinical supervision, and maintains all continuing education requirements.
  • Performing additional duties as assigned to support our mission.
